Practice Radius Template – A copy of this (which should have been completed earlier in the process) should be sent to Scottish Government.

Grant Offer Letter – Terms of the grant which must be signed by all applicants.

First Tranche Payment Template – Request for the first tranche payment to be made (80%) including information on the number of surgeries

Grant Offer Covering Letter – Health Board to send to applicant and confirm legally enforceable obligations.

NHS Board Due Diligence Confirmation – Health Board to confirm they are content with the grantee(s), all the documents produced and happy for the grant to proceed.

Second Tranche Payment Template – Health Board to send request to Scottish Government once certain conditions are met (e.g. practice has cause to be registered at least 1,500 NHS patients per grant-aided surgery and at least 80%+ of total practice’s income deriving from GDS).

Annual Report Template – Complete every year for the 7-year SDAI grant period.

Annual Report Confirmation – Confirm monitoring procedures are in place to the Scottish Government.
